Direct Deposit Transfer Form – Quick Guide 

If you receive regular direct deposits, like paychecks, dividends, or government payments, you’ll need to let the organization sending those funds know if your bank account information has changed.

This form makes it easy! Just:

  1. Fill in the name and address of the company or agency that sends your deposit.

  2. Provide your new account details.

  3. Sign the form. 

  4. Submit it to the company or agency to ensure your deposits go to the correct account moving forward.

Automatic Payment Transfer Form – Quick Guide

Use this form to switch your automatic payments to your Citizens State Bank checking account.

Here’s how:

  1. Fill out this form for each automatic payment you want to transfer.

  2. Attach a voided check from your new Citizens State Bank account.

  3. Send the form to the company handling your payment (you may also be able to make the change on their website or billing statement).

Don’t forget: It may take a little time for the change to take effect, so keep an eye on your first few payments.
